- Original and 2 copies of your Tournament Team Roster with original parent signatures. Only the Tournament Team Roster will be accepted.
- Proof of Age (original or copy of birth certificate, passport or Travel League player pass)
- Contact Form
- Signed Medical Release Forms for all players.
